Application Forms

The processing of an application to the Bishop Francis A. Quinn Schools require that parents and/or guardians submit the following documents to the Admissions Office with a non-refundable registration fee of $175.

(Applications will not be processed without registration fee.)

  • Application Form

  • Emergency Form

  • Student Questionnaire

  • Teacher / Conselor Recommendation

  • Copy of latest report card(Please Note-All incoming 10th, 11th, & 12th Graders must submit an official transcript)

  • Copy of Immunization Records

  • Signature Form/Acknowledgment of Rules and Regulations Form

  • Sports Contract

Applications received after the Registration deadline will be processed on a space-available basis.

After the Admissions Committee reviews your application, you will be issued a tentative acceptance letter.  Final acceptance comes after your 2006-2007 tuition contract is signed.
